Description

This presentation will show pharmacists how to see and review data contained within patient Flash Glucose Monitoring devices (phone apps) to identify issues with glycemic control. Utilizing the clinical targets for continuous glucose monitoring data, the participants will learn how these reports can be generated and shared by patients using their own devices. Proper interpretation of this data can provide rationale for pharmacist-led interventions and encourage a collaborative relationship for patients and pharmacists. Follow-up of patients can be done very effectively using a virtual sharing of FGM data, and provide evidence for successful interventions. Lessons learned from report sharing with pharmacists can integrate into daily practice to ensure safe and effective diabetes interventions during COVID times and beyond.
