I acknowledge the contagious nature of Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend preventative measures, including wearing masks (when not eating or drinking) and social distancing.

I further acknowledge that EnsembleIQ and Future Leaders in Convenience are relying on Des Moines Marriott Downtown, located at 700 Grand Ave, Des Moines, IA 50309 to implement and maintain preventative measures to reduce the spread of Coronavirus/COVID-19.

I further acknowledge that EnsembleIQ and Future Leaders in Convenience cannot guarantee that I will not become infected with Coronavirus/Covid-19. I voluntarily seek to attend Future Leaders in Convenience and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all recommended procedures detailed on the event website, and shared at the event through signs and other means, as well as any state or local rules or guidelines in place at the event at that time.

I attest that:

• If I am experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell within 14 days prior to the event start, I will not attend the event.
• I will not have traveled internationally within 14 days prior to the event start date.
• If I believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 within 14 days prior to the event start date, I will not attend.
• If I am diagnosed with Coronavirus/Covid-19 and not yet been cleared within 14 days prior to the event start date as non-contagious then I will not attend the event.

I hereby release and agree to hold EnsembleIQ and Future Leaders in Convenience, Des Moines Marriott Downtown, located at 700 Grand Ave, Des Moines, IA 50309, and all suppliers, harmless, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act by EnsembleIQ and Future Leaders in Convenience, or that may otherwise arise in any way in connection with attending and participating at Future Leaders in Convenience. This liability waiver and release extends to the owners, partners, employees, contractors and suppliers retained by EnsembleIQ to produce Future Leaders in Convenience. The foregoing is subject to change based on any Federal, state or local changes or recommendations.
